Position Summary

Functioning as part of the coordinated service delivery team, the Outreach Worker works proactively with new and existing clients, offering a range of support and recovery services, within as existing service plan and on an as-needed basis. The Outreach Worker is a relationship-builder and advocate and provides services in a non-judgmental and culturally appropriate manner, helping clients to build new skills and achieve their goals.
 
Qualifications
 
Education
Valid Drivers’ license and access to a reliable vehicle
 
Service Delivery Skills, Expertise and Experience
Minimum of 2 years’ experience working in community, preferably with the program's target population
Experience providing outreach with hard to hard-to-reach populations
 
Critical Skills
Proven knowledge and ability in relationship-building, critical thinking, facilitation, decision-making, problem-solving, and conflict-resolution skills
 
Focus on Results
Demonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes
 
Alignment with Organizational Values
Commitment to the principles that guide Mount Carmel Clinic, as reflected in the history, vision, mission, and values
 
Indigenous Understanding
Knowledge and understanding of Indigenous culture and traditions and the impact of colonization on the health and well-being of Indigenous individuals and families

Proven ability to work within an interdisciplinary team environment with respect, professionalism, cooperation, sensitivity, and cultural awareness

Communication
Excellent verbal and written communication skills and proficiency in the use of computer systems
 
Assets
Post-secondary education in a Social Services field
